As a  Licensed Practical Nurse   your work has real weight. You’ll serve older adults in Allegheny County—not in a hospital, but in their homes, communities, and at our Day Health Centers. You’ll be the face they trust and the steady hand they rely on. Provide hands-on personal and clinical care (wound care, medication administration, respiratory support) Monitor vital signs and report changes to the RN or IDT Assist with medication box preparation and physician order follow-up Maintain accurate documentation and communication Help Participants remain safe and supported in their homes Hands-On Impact – You make a visible difference every day. Team Support – You work closely with RNs, physicians, and social workers. Respectful Culture – You’ll be seen, heard, and supported by a team that cares. Active PA LPN license Minimum one year of experience  with the elderly and or frail. CPR certification Valid driver’s license, reliable  vehicle, and auto insurance This is more than a nursing role.
