This role includes triaging symptoms, providing education on diabetes, hypertension, hyperlipidemia, and weight management, coordinating care with providers and cross functional teams, and documenting patient care accurately in a telehealth setting.For you, caring for people with chronic conditions like diabetes and obesity isn’t a job, it’s a calling. What you’ll doDeliver patient centric health coaching that integrates medical needs with patient preferences.Triage patient reported symptoms and side effects according to internal protocols, escalating to Providers when appropriate.Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight management care and education via chat, phone, and video.Coordinate care seamlessly between patients, providers, support teams, and cross-functional clinical team members.Create consults that align with 9amHealth’s internal protocols for Providers.Document patient care accurately and consistently to meet internal standards.What you’ll needQualificationsStrong empathy, communication, and patient-centered care skillsBachelor of Science in Nursing (BSN) requiredActive Registered Nurse license in good standing, with ability to obtain additional licensure if neededCertified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified3+ years of direct patient care in an ambulatory or telehealth settingAdaptable, self directed, and comfortable working independentlyProficiency with digital tools for care deliveryFlexibility to participate in rotational weekend or holiday coverageA HIPAA compliant, distraction free workspaceThis role offers a salary of up to $100,000, depending on experience.What you can expect at 9amHealthAt 9amHealth, you'll join the exciting environment of a startup with the stability of highly experienced founders that have successfully built a business from scratch.
