Evaluates the effectiveness of the plan in meeting established care goals; revises that plan as needed to reflect changing needs, issues, and goals.  Educates the patient/family, promotes self-management, and empowers patients/families to achieve maximum levels of wellness and independence through coaching techniques, like motivational interviewing, through telephone calls and in-office care management visits Develops collaborative working relationships with insurance case managers and other key parties working with a particular patient/family case; negotiates on behalf of the patient with third parties for cost-effective, high quality services, and to maximize the efficient use of resources  Is primary contact with Beth Israel Deaconess Physician Organization to ensure timely case management of patients in accordance with risk-based contracts with payers. In conjunction with Medical Director, reviews related reports to improve performance on productivity, cost and quality measures.  Work two - three sessions per week as a Triage Nurse and the equivalent to two sessions per week seeing patients for face-to-face Care Management. Monday – Friday Schedule - Excellent Compensation and benefits PackageBachelor of nursing with at with at least three years of experience preferably in an ambulatory setting with Family Practice focus; Care Management experience preferred Massachusetts Nursing license – RN, Experience with telephone assessment and crisis management; excellent communication and documentation skills  Contact Jerome at Olesky Associates for More Information
