FUNCTIONS: 40%- Provides culturally sensitive home visitation services for families that promote positive parent-child interaction, child developmental skills, and healthy and safety practices through the use of curriculum and other educational materials in accordance with best practices model standards. Provides information, referrals, and linkages to available community resources. Links families to a medical provider to assure optimal health and development. 10%- Administers developmental assessment tools and screenings to monitor progress of child health and development and parent-child interaction skills in accordance with timelines 5%- Works with families to develop individualized  family service plans and to meet stated goals. 30%- Maintains accurate and complete electronic, case files, and program records in accordance with timelines. 5%- Facilitates relationships and coordinates service with program collaboration partners and other community agencies. 5%- Participates in supervision on a regular basis and participates in required trainings. 5%- Performs all other duties as assigned   The above is intended to describe the general content and functions of this job. MINIMUM EDUCATION: Required minimum education: Bachelors Degree in Social Work, Early Childhood Development, Teaching, r Nursing, or related human services field LICENSURE/CERTIFICATIONS: Required: Must obtain home visitor credential during orientation period and maintain credential throughout employment MINIMUM QUALIFICATIONS: Minimum years and type of experience: Preferably two years of recent experience working in the community experience with an emphasis on home visiting. Strong understanding of child development and community resources. Other knowledge, skills, and abilities required: Ability to write effectively and speak clearly. Reliable transportation, valid driver’s license, and proof of insurance. Other knowledge, skills, and abilities preferred: Ability to work well with and relate effectively with co-workers, staff, or other service providers, parents, young children, and community groups. WORKING CONDITIONS: Periods of high stress and fluctuating workloads may occur. General Office environment: works generally at a desk with well-lighted, air-conditioned office area, with moderate noise levels. May be exposed to physical altercations and verbal abuse. May be exposed to adverse weather conditions: cold, hot, dust, wind, etc. Required to car travel to off-site locations, occasionally in adverse weather conditions.  Long-distance or air travel as needed, not to exceed 10% travel. May be exposed to limited hazardous substances or body fluids. Potential exposure to infectious organisms.
