You should excel in creating and managing care plans, navigating behavioral health resources, and offering clinical guidance that supports both short- and long-term outcomes.   Schedule: Monday-Friday 9am-6pm PST\n Responsibilities:<ul> <li>Deliver coordinated, family-centered virtual Care Management by phone that improves behavioral health outcomes for pediatric members.</li> <li>Develop personalized, goal-oriented care plans in collaboration with families and our multidisciplinary team.</li> <li>Support members through behavioral health treatment plans, transitions of care, and episodes of acute behavioral health crises.</li> <li>Help families navigate behavioral health systems, treatment options, benefits, and community-based resources.</li> <li>Partner with local pediatricians, behavioral health specialists, schools, and other care team members to ensure aligned, coordinated care.</li> <li>Provide compassionate, longitudinal support to families through continuous engagement and advocacy.</li> <li>Coordinate resources that address clinical, behavioral, and social determinants of health impacting the child and family.</li> </ul> Required Qualifications:<ul> <li>Bachelor of Science in Nursing (BSN) required</li> <li>Must reside in a compact NLC state</li> <li>Active Compact RN license in good standing with the nursing board of their state</li> <li>Active California Nursing License</li> <li>Willingness to become licensed in multiple states</li> <li>Current CCM Certification preferred</li> <li>5+ years Nursing experience</li> <li>2+ years experience working in Behavioral Health Pediatric setting</li> <li>Be able and willing to work until shift schedule: Monday- Friday, 9am- 6pm PST</li> <li>Comfortable managing a wide range of pediatric behavioral health conditions including anxiety, depression, ADHD, autism spectrum disorders, and trauma-related diagnoses.</li> <li>Ability to manage volume while maintaining quality and a high standard of care.</li> <li>Flexible and adaptable in a fast-paced, rapidly evolving care delivery model.</li> <li>Familiar with HIPAA and committed to upholding the privacy and security of patient information.</li> <li>Excellent verbal and written communication skills; able to explain clinical concepts in parent-friendly language.</li> <li>Technologically competent and able to work across multiple electronic systems.</li> <li>Demonstrated success working in multidisciplinary teams to coordinate care.</li> <li>Deep understanding of cultural and socioeconomic factors influencing pediatric behavioral health.</li> <li>Exceptional attention to detail and documentation quality.</li> </ul> \n  The United States new hire base salary target range for this full-time position is:   $73,700 - $103,180 + equity + benefits     This range reflects the minimum and maximum target for new hire salaries for candidates based on their respective Zone. Compensation structures and ranges are tailored to each zone's unique market conditions to ensure that all employees receive fair and competitive compensation based on their roles and locations.
